To enroll in your medical (including prescription), dental, vision, optional life Insurance, MetLife Legal plan, critical illness insurance, accident insurance or the FSAs or HSA plans, log on to Benefits Self Service at HologicBenefits.com between March 3 – March 14, 2025. For most benefits, once this Special Enrollment closes, your only opportunity to make changes during the plan year is within 31 days of a “qualifying life event,” such as a marriage, divorce, or the birth or adoption of your child.
Steps to Enroll
Log on to Benefits Self Service at HologicBenefits.com from March 3 – March 14.
- Register with your social security number, date of birth, zip code and company code “hologic” (all lowercase) to create your username and password.
- Alternatively, you may also utilize the MyChoice mobile app to make your open enrollment elections on-the-go. Download the MyChoice mobile app to your smartphone today. Be sure to obtain your “Access Code” from the home page on Benefits Self Service first.
Important! Dependent Coverage Requirements
Be prepared to provide proof of your relationship to any dependent you are adding to your medical, dental, vision, optional life insurance, accident, or critical illness insurance plans, such as a birth certificate (for a child), or marriage certificate (for spouse/partner), or last year’s tax return (child and/or spouse/partner) in addition to having social security numbers available.
